Oregon Senior Placement Advisory
Strategic Placement Process
With Your Advisor
Expert-guided, clinically grounded, and built around your loved one’s specific needs — from first conversation to long-term advocacy.
Our Core Process
Every family’s situation is unique. Our advisory services are built around your loved one’s clinical, behavioral, and personal needs — not available beds.
01
Needs Assessment
We begin with a detailed assessment of medical stability, cognitive presentation, mobility, behavior, and long-term sustainability.
02
Care Evaluation & Strategy
We determine the right level of care and create a placement strategy based on support needs, financial structure, and long-term fit.
03
Provider Alignment
We identify communities and care providers that are properly equipped to meet the client's needs safely and appropriately.
04
Tour Preparation
Families are prepared to evaluate safety, staffing, leadership, and care quality before stepping into any community.
05
Transition & Ongoing Advocacy
After placement, we help coordinate the transition and remain available for continued support as needs evolve.

Building YourLong-Term Living Plan
To create a placement strategy that holds up over time, we review the following areas with every family. These are not optional — they are the foundation of any sustainable placement recommendation.
Financial Structure
Private pay or Medicaid status, monthly budget, and all income sources. This determines the realistic universe of placement options from the outset.
Asset Positioning
Real estate, savings, and liquidity that may impact long-term sustainability. We need to understand what resources exist and how long they can support the plan.
Location Preferences
Geographic priorities and proximity to family or support systems. Location affects both quality of life and the practical sustainability of family involvement in care.
Medical Profile
Diagnosis, physical care needs, cognitive presentation, and mobility level. This shapes every aspect of the placement strategy — from licensing requirements to behavioral fit.
Care Licensing Fit
Placement alignment based on current and projected care needs. Not every provider is licensed for every level — we match your loved one to what they are actually licensed and equipped to handle.
Stability Horizon
Planning not only for today, but for progression over time. The right placement today must remain appropriate — or have a clear transition path — as needs change.
Important Note Regarding Medicaid
If you are currently receiving Medicaid long-term care benefits, placement options are often limited to providers contracted through state systems. Please read the guidance below carefully.
If you are currently receiving Medicaid long-term care benefits, placement options are often limited to providers contracted through state systems. In these cases, your primary coordination will typically occur through your local Senior & Disability Services office or the Oregon Department of Human Services (DHS).
We are always willing to provide general guidance and clarity; however, direct placement advisory may be limited depending on Medicaid status and case management structure.
We encourage families working within Medicaid to remain in close contact with their assigned case manager through the following resources:
